increase in the number of clients and a decrease in the number of employees\u2014is putting many healthcare organizations in a bind. How should they organize their care services? Anne Bosman, director of Accolade Zorg, enlisted Turner to develop a new strategy. What was the issue that led to Turner being brought in? And what made it so complicated? <\/strong><\/span><\/p><p>\u201cDo you have an hour?\u201d Anne Bosman, who has been the CEO of Accolade Healthcare for over six years now, can\u2019t help but quip when asked what developments the healthcare group will face in the coming years. He cites demographic trends\u2014with the number of seniors set to double by 2040\u2014and the opposite trend in the labor market, where the number of students being trained for careers in Healthcare is actually declining rapidly. On the one hand, we\u2019re aging; on the other, there aren\u2019t enough new healthcare workers coming into the field. Within the sector, this is known as the \u201ccare gap.\u201d \u201cChoices have to be made,\u201d says Bosman. \u201cThe way Healthcare is structured now, it will be unsustainable for the Netherlands in the long run. Because there are other choices to be made\u2014in Education, for defense, for the climate\u2014we can only spend our money once.\u201d<\/p><p>So it\u2019s high time for a new strategy, according to Bosman. The previous strategy had just been formulated when he joined the company in 2017. It was a useful document that served him well, Bosman says, but changes are happening at a rapid pace. And given the importance of making the right choices in this complex landscape, he decided last year to bring Turner on board.   <\/p><p>Accolade operates in the fields of nursing home care, assisted living facilities, and home care. The care group has six locations and serves a diverse range of clients, including people with dementia, people with somatic care needs, and people with acquired brain injuries. In addition, Accolade provides home care in two regions. In total, approximately 800 people work for Accolade, which has revenue of about 50 million.   <\/p><p>Looking back, you could say that the healthcare sector was still in relatively calm waters in 2017. \u201cYou could already see the healthcare gap coming back then, but it wasn\u2019t an urgent problem yet. Now it is. The problem threatens to become incredibly serious. If you do the math, by 2040 one in every three or four workers would have to be in the Healthcare sector. Of course, that\u2019s completely impossible. But the problem gets more complicated every year.\u201d <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-7da5938 elementor-widget elementor-widget-text-editor\" data-id=\"7da5938\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><span style=\"color: #084a87\"><strong><img decoding=\"async\" data-src=\"https:\/\/wordpress-1053401-6586567.cloudwaysapps.com\/wp-content\/uploads\/2023\/06\/Groei@3x-150x150.png\" class=\"alignleft wp-image-27194 size-thumbnail lazyload\" src=\"data:image\/gif;base64,R0lGODlhAQABAIAAAAAAAP\/\/\/yH5BAEAAAAALAAAAAABAAEAAAIBRAA7\" alt=\"\"  ><noscript><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-27194 size-thumbnail\" src=\"https:\/\/wordpress-1053401-6586567.cloudwaysapps.com\/wp-content\/uploads\/2023\/06\/Groei@3x-150x150.png\" alt=\"\" width=\"150\" height=\"150\"><\/noscript>2. What led to the breakthrough? In terms of content, change management, and\/or project management? <\/strong><\/span><\/p><p>After extensive consultation with all stakeholders, a new 18-page strategy was drawn up. It is a fairly detailed document, with all the major themes mapped out over the next five years. \u201cThink of it as a list of all the major issues for the coming period,\u201d Bosman explains. \u201cWe\u2019ve incorporated the themes into a multi-year program with as concrete a plan as possible\u2014including the priorities\u2014so that the strategy doesn\u2019t just exist on paper but will actually be implemented.\u201d   <\/p><p>The labor market is, of course, one of the key priorities in the strategy. How do you ensure that potential employees choose you over another employer? And how do you ensure that the people who already work for Accolade stay? One of the aspects that should help Accolade achieve this is its Christian identity, says Bosman. \u201cWe are an organization that provides healthcare from a Christian perspective. Of course, there are different ways to view this, but as far as we\u2019re concerned, it\u2019s a USP\u2014a <em>unique selling point<\/em>. We are a Christian organization and want to remain so. This works both ways: it\u2019s appealing to our clients, who feel at home with us, and it can also help us in the job market. But we do give that identity substance in a modern way. \u201cOur Christian identity doesn\u2019t come with a \u2018brussels sprouts\u2019 vibe.\u201d         <\/p><p>Another key focus of the new strategy is innovation. \u201cIn the near future, we will, by definition, have too few people to provide the same level of healthcare as we do now,\u201d Bosman explains. \u201cSo we have no choice but to focus on innovation, both technologically and socially. For example, we\u2019re working to integrate family caregivers and volunteers into our care process. Viewed from a distance, that sounds fairly straightforward: you ask one of the children to help with healthcare for their parents. But of course, that has an impact on our own employees, who have undergone years of training. They will certainly have an opinion about it. Then, of course, there\u2019s the legal side: is this even allowed? How can you encourage informal caregivers to commit? After all, we\u2019re not an employer. So we\u2019ll have to transform the way we work.\u2019  <\/p><p>It\u2019s also not easy to ask informal caregivers for help. \u201cWhen residents end up in a nursing home, it\u2019s often the result of an intensive process that came before. Those informal caregivers have sometimes been pouring all their energy into providing healthcare for years. Can you then immediately ask them to contribute? That\u2019s really a dilemma.\u201d    <\/p><p>Turner\u2019s role in this process was primarily to help make decisions. \u201cAs an organization, you simply have to deal with limitations,\u201d says Bosman. \u201cThere are only 24 hours in a day, and you want to spend some of those hours sleeping. Accolade has both general activities and a number of niche areas. So the number of initiatives has to remain limited. That also helps get the organization on board. I need my employees to come up with ideas on how we can organize Healthcare differently; I can\u2019t figure all that out from behind my desk or with just a few colleagues here in the hallway. It was about making choices, setting priorities, and then getting down to business. That doesn\u2019t mean we\u2019re just going to abandon our niche areas. We also have a responsibility. We provide geriatric psychiatry; we have an acquired brain injury (ABI) unit for people with acquired brain injuries, and a long-term intensive neurorehabilitation (LIN) unit. These are small target groups. Our LIN unit is one of only three specialized facilities in the entire Netherlands\u2014we\u2019re quite proud of that. But in terms of volume, it\u2019s very small, though still important.\u201d  <\/p><p>The strategy has helped shape our thinking, says Bosman. \u201cUp front, we had actually expected more radical choices. In some cases, we haven\u2019t made those choices yet. Take home health care, for example. It\u2019s an important service from a societal perspective, but we\u2019re having trouble making it financially viable. We\u2019ve implemented a few measures there that have helped us break even, so we don\u2019t need to inject any additional funds.\u201d <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b57b398 elementor-widget elementor-widget-text-editor\" data-id=\"b57b398\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><span style=\"color: #084a87\"><strong><img loading=\"lazy\" decoding=\"async\" data-src=\"https:\/\/wordpress-1053401-6586567.cloudwaysapps.com\/wp-content\/uploads\/2023\/05\/Meerdere-richtingen@3x-150x150.png\" class=\"alignleft wp-image-26294 size-thumbnail lazyload\" src=\"data:image\/gif;base64,R0lGODlhAQABAIAAAAAAAP\/\/\/yH5BAEAAAAALAAAAAABAAEAAAIBRAA7\" alt=\"\"  ><noscript><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-26294 size-thumbnail\" src=\"https:\/\/wordpress-1053401-6586567.cloudwaysapps.com\/wp-content\/uploads\/2023\/05\/Meerdere-richtingen@3x-150x150.png\" alt=\"\" width=\"150\" height=\"150\"><\/noscript>3. How did the execution come about?<\/strong><\/span><\/p><p>Developing the new strategy has been a very meticulous process, Bosman explains. \u201cWe first took a few different snapshots. What is the current situation? How is the outside world changing? What lies ahead in terms of demographics, regulations, and the ministry\u2019s expectations? What does the internal situation look like? What are we happy with? What aren\u2019t we happy with? Once you have the answers to those questions, you start peeling back the layers and weighing the importance of all the different developments. You try to bring logic and coherence to it. That\u2019s how it continues, step by step, from a broad to a detailed perspective. And then the process narrows down: what do we agree on, and what do we still need to discuss? That way, the number of questions keeps getting smaller, until you say: \u201cGuys, we\u2019ve got it. Let\u2019s wrap it up.\u201d <\/p><p>Turner has seen the management team grow closer together throughout the process. \u201cAt first, I noticed a tendency among the managers to view things primarily from their own perspective,\u201d he says. \u201cAnd that has changed as a result of the new strategy. They now also consider things from the perspective of others and of the foundation as a whole.\u2019 A good example was the discussion about domestic help, which took place during one of the first meetings. \u201cSome were in favor of providing housekeeping services. They said that aligns very well with our Christian identity. We should help people with that, possibly with the help of volunteers. Others said: \u2018No way, we shouldn\u2019t do that; that service isn\u2019t profitable.\u2019 The debate was conducted based on different values. That was an important aspect of the process. Choices had to be made, but they had to be based on a clear vision of Accolade Healthcare\u2019s future.\u2019   <\/p><p>The strategy runs from 2023 through 2027, Bosman explains. \u201cBut the strategy is so concrete\u2014especially for 2023\u2014that it has essentially become the annual plan. We\u2019ve defined what we want to have achieved by the end of the year. Turner played a key role in that. There are plenty of agencies that can help develop a strategy\u2014any consultant can create nice visuals\u2014but when it comes to actually executing it, that\u2019s where Turner really stands out. Of course, Turner also provided all kinds of tools and played a role in brainstorming, reviewing, refining, and formulating the strategy, in collaboration with the internal management team consisting of a regional manager, the executive secretary, a senior quality assurance officer, and the controller. Thanks to their input, we\u2019ve been able to truly make that strategy our own. I really appreciated that sense of collaboration, where both sides joined forces.\u201d <\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-4f07111 elementor-widget elementor-widget-text-editor\" data-id=\"4f07111\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><span style=\"color: #084a87\"><strong><img loading=\"lazy\" decoding=\"async\" data-src=\"https:\/\/wordpress-1053401-6586567.cloudwaysapps.com\/wp-content\/uploads\/2023\/05\/Groep@3x-150x150.png\" class=\"alignleft wp-image-26292 size-thumbnail lazyload\" src=\"data:image\/gif;base64,R0lGODlhAQABAIAAAAAAAP\/\/\/yH5BAEAAAAALAAAAAABAAEAAAIBRAA7\" alt=\"\"  ><noscript><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-26292 size-thumbnail\" src=\"https:\/\/wordpress-1053401-6586567.cloudwaysapps.com\/wp-content\/uploads\/2023\/05\/Groep@3x-150x150.png\" alt=\"\" width=\"150\" height=\"150\"><\/noscript>4. Are there benefits for all stakeholders\u2014customers, shareholders, employees, management, and society? <\/strong><\/span><\/p><p>The new strategy is explicitly aimed at ensuring the continuity of healthcare. This, of course, primarily benefits the clients of all Accolade\u2019s services, from home care to niche services. For employees, it is beneficial that the strategy focuses on enhancing quality and expertise, for example in the field of elder care. In doing so, Accolade also supports other providers who do not have this specialty in-house. Accolade is also increasingly seeking partnerships with primary care physicians, as they, too, are seeing more and more older adults with age-related health issues in their practices. \u201cOur strategy also offers benefits to other parties in the care chain,\u201d Bosman notes.     <\/p><p>The funders, such as the healthcare administration offices, are benefiting from improved financial discipline\u2014for example, in the home care operations\u2014which is now bringing Accolade closer to breaking even.<\/p><p>At the same time, Bosman is looking into whether he can reduce the number of contracts with health insurers. \u201cWith some insurers, we have fifty clients; with others, sometimes only five. Handling all the paperwork for that is quite labor-intensive. Perhaps we should just say: \u2018We don\u2019t have a contract with this insurer; you might be better off going to another provider.\u2019 That\u2019s an unpleasant reality, but it makes sense for the sake of continuity. Ultimately, this will allow us to reduce our administrative costs. What was the biggest dilemma, and how did you handle it?<\/strong><\/span><\/p><p>Bosman believes the biggest dilemmas lay hidden in the choices that had to be made. \u201cFor example, we really had to figure out what home care entails. In the past, it mainly involved putting on compression stockings, but those days are truly over. What is complex home care? What are we already doing? And how far do we want to go with it? When do you have to say \u2018enough\u2019?\u201d Home care and nursing care are becoming increasingly intertwined; these two worlds are merging more and more. Healthcare provided in the home is becoming more intensive. So how should we approach this? A concept has been developed that clearly maps out the broad range of home care services\u2014from mild to highly complex\u2014including what we currently do and don\u2019t do within that scope.
