The Right Healthcare in the Right Place
Breakthrough in First-Line Healthcare
Spreekuur.nl is a great example of how digital healthcare makes it possible to provide the Right Care in the Right Place. With this triage and diagnosis app, patients can take the first steps in primary care on their own. Turner is helping to achieve these—and other—breakthroughs in providing the Right Healthcare in the Right Place.
Spreekuur.nl
Spreekuur is an app that provides access to healthcare advice at any time of day. Patients perform their own triage by answering questions (based on NHG guidelines and NTS standards). and, when applicable, uploading photos. This ensures that the primary care physician is well-prepared and can make a diagnosis more quickly. In this case, it became apparent that:
- Using the Spreekuur.nl app, approximately 32% more patients can be treated per hour compared to in-person and telephone consultations.
- The time a healthcare professional (triage nurse, primary care physician, assistant) spends with a patient is 8–11 minutes shorter in the consultation process. This is 36–50% shorter than an in-person consultation in the standard process.
- Of the consultations during office hours , a large majority would normally have been in-person consultations (according to general practitioners). Conducting consultations digitally could save approximately 100 FTEs in the Netherlands, assuming full adoption and efficiency.
- The NPS score of 54 is very high, despite the fact that some of the healthcare provider’s work is being shifted to the patient.
- The total turnaround time for the patient—from triage to diagnosis—can be shortened through the digital process.

"The Right Healthcare in the Right Place" in Practice
Healthcare in 2020 faces many challenges, such as changing healthcare needs during the COVID-19 pandemic, as well as a growing demand for healthcare in general. A new way of thinking and working is needed to keep healthcare accessible, high-quality, and affordable in the long term. This requires collaboration and knowledge-sharing with creativity and courage. The Right Care in the Right Place (JZOJP ) is a trend we would like to see accelerate. This trend is based on three pillars: preventing the need for (more expensive) healthcare (1), shifting healthcare and organizing it around people (2), and replacing healthcare by implementing smart healthcare and e-health (3).
A striking example of innovation is the provision of primary care online. This allows patients to receive triage and diagnosis from home, rather than over the phone or in person at the HAP (shift in the delivery of healthcare). This makes healthcare more efficient and therefore less expensive, and also benefits the patient. Turner supports these types of initiatives by helping them with Strategy Execution.
Turner's 4-Step Approach

Step 1: Data Normalization
We provide quantitative insight into the processes: we use the numerous data logs from the digital triage and consultation system and link them together in a secure and reliable manner.
Step 2: Insights from Hypotheses
We test hypotheses. For example:
- number of patients to be treated per hour
- Time spent by healthcare professionals per patient
- Number of in-person/telephone consultations provided
- NPS/Patient Satisfaction Scores
- patient turnaround time
Step 3: Business Case
We are preparing a business case for health insurers
Step 4: Recommendations
We provide recommendations in the areas of data, process optimization, and management
